If the author is also the cover artist/cover photographer/illustrator/composer, etc. how is that credited?

One credit for the main author and a second listing for the other thing they did? A separate listing for each thing they did?

Same question for secondary authors. If the illustrator is also the cover artist, for example?

I've got a book in which the main author also took the photo used for the cover. I have several books illustrated by the main author.

In my cases where the editor of an anthology is also a contributor, I've been applying them twice to the work, once for each role. If I should stop doing that, lemme know. :-)

People do it differently. I generally just credit what I view as the person's primary role, though in the case >2 omargosh: posits, I'd probably do the same.
